All requests for registration refund and or player withdrawal must be made in writing using our Regions Refund Request Form.

TERM AND CONDITIONS:

1. Only one form per child.

2. A child may not play in AYSO games or practices once this form is submitted.

3. Refunds are normally processed within 6 to 8 weeks. Incomplete forms could take longer.

4. All registration refunds will be adjusted as follows

• For any discount received at time of registration
• A $17.50 processing fee will be deducted from all refunds.
• A $20 insurance fee will be deducted if form is received after August 1st.
• $25 will be deducted if player has taken possession of uniform
• The AYSO National Membership fee (currently $20) is NON-REFUNDABLE

5. No refund will be given if form is received after September 30th of the registration year.

6. AYSO is not responsible for forms lost in the mail.
